Requesting Public Documents from the Health Department

To request public documents from the City of Newton's Health Department, please complete the Public Document Request Form and mail it to the Health Department at 1000 Commonwealth Ave., Newton, MA 02459 or fax it to (617) 552-7063. There is a nominal fee ($.20 per page) for the photocopying of more than 10 pages. The results of your request should be available several days after it is received. You may call the Health Department at (617) 796-1420 for the results of your request.
